St.William the Abbot Parish, as well as all of the parishes in the Diocese of Rockville Centre, needs to have a record of all those involved in ministries in their church community.
Volunteers in specific ministries will be asked to provide additional information, instructions is included with the form package so you can identify those form you would need for that specific Ministry.

- Once you have completed these form they can be dropped off at the rectory or mailed. If you are mailing the form or dropping them off, please place them in an envelope and address them to:
